New Coordinated Entry Process

After many stakeholder meetings and significant research, TJACH has identified a new tool for assessing households for homelessness resources. The new Coordinated Entry Packet includes:

1) A cover sheet identifying the components of the Coordinated Entry Packet

2) Pre-screening questions to help service providers determine whether they should complete a Coordinated Entry Packet

3) Demographic questions required by funder agencies

4) The Vi-SPDAT for individuals

5) the Vi-SPDAT for families

6) Release of information form to allow for appropriate information-sharing between service providers working with the household to resolve their housing crisis

7) Permission form for collecting data using HMIS
